Making difficult choices? What you can expect

We will aim to ensure you understand everything and feel comfortable every step of the way. To help with this we have set out the first of the steps to explain the process of putting a package of care in place.

 Step 1 Our Team will receive your enquiry either by phone or by E Mail. We will discuss your specific care requirements and get to know what matters to you most. We will also provide information on the services that are available to you, as well as an outline of costs.

Step 2 We will arrange to visit you at home and carry out a full assessment of your care needs, including any risk to ensure your safety. A fully costed care package will be offered in writing following this visit.

Step 3 We will finalise your Care and Support Plan with you and your family, making sure you agree with it and are happy to go ahead. This Care and Support Plan will be very personal to you aimed at helping you with your independence and getting the most out of life.

Step 4 We will identify suitable Care and Support workers in-line with your care plan and arrange for you to meet them. If any further training is required to support your care requirements, we will arrange that.

Step 5 Your Care and Support service begins

And finally ……. we understand that in a crisis you need us to respond more quickly, where possible allowing a care package to start immediately. At such times we will simplify our Care and Support Planning process, carry out an immediate assessment and enable a faster and if possible a same day response.
